I sometimes ask if they want to know for real or just being polite.

"How are you doing" Is a call-and-response looking for a "yeah, how you doin'?"
"How are you feeling" is an inquiry about your health.
That's why my go-to is. "Well I'm here..." and let them make of it what they will.
