It took about an hour and a half and a lot of experimentation to get something I was reasonably happy with. Ultimately what worked was multiple inpainting passes followed by a few img2img passes using Latent Couple. I upscaled using the Ultimate SD Upscale extension to stable-diffusion-webui, which has a bunch of different options that go well beyond the normal img2img upscaling options and even surpass the new txt2img hires fix options. Due to the complexity it is pointless to share generation info, but for the inpainting & latent couple settings for masks over her breasts "a pair of vaginas" seemed to work best.
Rendering at larger sizes can also make it more likely SD will generate multiple subjects. You can use the Regional Prompter or Latent Couple extensions for stable-diffusion-webui to control the subjects separately.
You can also generate subjects separately using txt2img, paste them into a single scene, then do a pass with img2img to make the image look seamless. That is about the hardest approach, though. ControlNet with OpenPose and region prompter are my go-to solution.
My apologies for the lack of diversity in the poses; it's because the LoRA was trained on pussy closeups, so even getting her face in the frame required a little work and a weight of 0.5 on the LoRA. I can do other poses if you want.

I was able to make this one work using inpainting with "mask only", by fooling the model into thinking her cleavage was a butt. Then I upscaled and did one more pass separately on her cleavage pussy and the rest of her.
I had very little luck using latent couple with this one, but I didn't try the Canny ControlNet trick, just OpenPose.
I didn't have much luck using just the OpenPose ControlNet. I tried adding another face between her legs just to see what it would do. Usually it would do nothing until I turned the ControlNet's weight way up, then it would produce a full human head, usually with its neck going down through the sheet or something. But in this one the second head appears to be protruding from her body.
I wasn't satisfied with the first result, so I kept trying. First I tried the SoftEdge ControlNet. That was a lot harder to work with and didn't produce good results. The breakthrough came when I tried using Latent Couple instead of Regional Prompter. It seems better suited to precision work like this. Next I'm going to try using just the OpenPose ControlNet to keep her in the same pose so that the Latent Couple mask is always in the right place. That won't require using GIMP to edit the Canny input.
Aside from the edited Canny input image, this just required one step, so I'll share the generation info. It's missing the Latent Couple config unfortunately.

I've been trying to do something like this for ages. I'm not quite happy with how it's come out so far, so I plan to keep trying. I used a "canny" controlnet and used PhotoShop to produce the control image, which is just outlines. Then when I got something that started to look close I switched to inpainting of just the crotch region. That resulted in some artifacts around the mouth, so I used img2img, which caused it to start looking more like a painting, so I had to mess with the prompt. Not much use in sharing generation info on this one because of the number of different steps.

I used this LoRA. It's for anime images, but with some effort I was able to make it generate realistic images using cyberrealistic_v32. It kept making blurry pictures, though, so I used Regional Prompter and Composable LoRA so that it wouldn't affect thetop half of the image. Her legs look a little weird, but I think it came out pretty good overall! Let me know what you think.

I didn't use hires fix on this one. Let me know if you want a high resolution version of anything I post in low resolution. It's potentially better than a regular upscale, even with an AI upscaler, because SD can add additional detail.
