My understanding is that Bill Lawrence, the creator of the show, planned it as a spin-off, but ABC didn't want that. It's why it was also called "Scrubs: Med School" in some places.

Scrubs: Med School is an okay name for a spinoff. It's in the same continuity and the same characters exist.
