Try MakeMKV in manual mode? You have to enable advanced and then it will show you the raw structure instead of trying to decide what are real titles for you.

May i suggest a dinosaur program? Try BadCopy Pro, make an image, then try MakeMkv. It's been a long time since i used a disc, my memory can be a bit hazy.
