Last name as in quantity. "I have 4 names and this is the last of them"
Oh... Wow.
Just imagining the context where it would be phrased like this - "Hi, good to meet you, my first name is John, and my last name is Smith."
Could have been a case of two people having the same first name and the one person wanted to clarify.
