Kids. Being married before you have kids is huge in some states and important in many. In my state unmarried father's have no rights to children even if they sign the birth certificate. Sure you can adopt, but that's far more expensive than a marriage certificate.

Protection in the case of breakup or divorce. You have rights to shared property in a divorce, you have no rights to anything you didn't buy or put in your name otherwise. You can sort of solve this with making a partnership and putting all assets into it, but it's not quite the same and far more complicated. Also if you aren't the breadwinner, there isn't really a way to ensure spousal support without a marriage.

Legal protections. You can't be compelled to testify against a spouse. While you can do things like medical power of attorney, you don't get it by default like marriage, which means you either need that document on hand at all times or in an emergency situation you could be prevented access or decision making authority until you provide documentation. There's also social security, you can draw on a spouse, but there isn't an equivalent, same for pensions that offer survivor benefits.

It's largely a problem with misusing specialty doctors and emergency rooms. There needs to be a decade long focus on more family medicine doctors and improving their workload and pay.
ER and urgent care doctors are trained in acute care and trauma. They aren't trained or meant to handle chronic disease or preventative care, they are meant to handle after the bad thing happens.

This largely works to explain it. I would assume an install package is a simplified set of plays and theories for a player to read and understand. Sanders didn't understand the package or wasn't prepared for talking about it.
Fixing the level system so you aren't punished for using major skills regularly is a really good change to the system. Managing a spreadsheet to level efficiently was always a huge negative.
It makes sense for things to level in an open world game where someone could encounter an area at level 1 or 10. In order to provide a reasonable challenge for the player coming to that area. What oblivion did wrong is it was far too global and there weren't sensible caps and floors on areas.
I don't see things changing without a coordinated strike and deaths from lack of care. I also don't think most healthcare workers are up for that.
